Important Consumer Information – Accessing Your Health Care Records & Filing a Complaint

In accordance with Texas Health & Safety Code Section 181.105 and House Bill 4224 (89th Regular Session)(effective September 1, 2025), you have the right to access your health care records and the right to file complaints with the appropriate regulatory authorities. Below are the steps you may follow:

1. Request Your Health Care Records From Us
If you would like to request a copy of your health care records, please contact our office using one of the following methods:

3. File a Consumer Complaint With the Texas Office of the Attorney General
